Misconduct Report.

The Tapestry is committed to creating and ensuring safe environments for worship, witness, and work and will not tolerate inappropriate behaviors or any form of leadership misconduct from its staff or volunteers. 

Concerns around leadership conduct or misconduct will be taken seriously, addressed, and where needed will be investigated promptly with integrity and fairness, prioritising concern for the well-being of complainants and other involved parties.  Information about allegations and complaints shall be kept confidential to the extent possible. 

Information obtained related to leadership conduct concerns or misconduct allegations, including identifying information about any individuals involved will not be disclosed publicly and only provided when and where that disclosure is absolutely necessary to investigate the incident, take corrective action, or otherwise required by law.   Names of any possible victims are kept confidential.  

To make a report, please fill out the form below. Your communication and identity will remain confidential. All reports are received by the Vice Chairperson of the Elder Board (currently Iris Lo) who may or may not be in contact with you.  If you wish to have her follow up with you, please indicate that in the report.
